STAR-TREC and TESAR trials: strengthening the evidence for organ preservation in rectal cancer

The adoption of total mesorectal excision (TME) has significantly reduced the risk of locoregional recurrence in patients with rectal cancer. However, this reduction comes at the cost of substantial treatment-related morbidity, including permanent stoma formation and long-term impairment of bowel, urinary, and sexual function that adversely impact quality of life (QOL) and functionality.1 As such, management has evolved toward risk-adapted, patient-centred strategies, including selective organ preservation that is oncologically safe and associated with better QOL.
