In the article that accompanies this editorial, Sherry et al17 report the results of the EXTEND randomized trial, showing that metastasis-directed therapy (MDT; 98% treated with radiotherapy) improved progression-free survival overall and across most histologic subgroups, except in breast and renal cancers. MDT was associated with immune activation, while detectable circulating tumor DNA before or 3 months after treatment predicted worse outcomes, supporting future strategies incorporating biomarker-guided patient selection and immune modulation in this setting.
