Perioperative FLOT: new standard for gastric cancer?
Mené en Allemagne sur 716 patients atteints d'un cancer de l'estomac ou de la jonction œso-gastrique de stade localement avancé et résécable, cet essai de phase II/III compare l'efficacité, du point de vue de la survie globale, et la toxicité d'une chimiothérapie péri-opératoire combinant fluorouracile, leucovorine, oxaliplatine et docétaxel (chimiothérapie de type FLOT) et une chimiothérapie combinant épirubicine, cisplatine et fluorouracile ou capécitabine (chimiothérapie de type ECF ou ECX)
The current standard of care for locally advanced resectable gastric cancer involves D2 resection with adjuvant treatment. However, there are geographical differences in the standard adjuvant treatment strategies, such as postoperative chemoradiotherapy in North America, postoperative chemotherapy in Asia, and perioperative chemotherapy in Europe, each based on pivotal phase 3 trials (Intergroup 0116, MAGIC, ACTS-GC, and CLASSIC trials). Several trials have since tested intensified adjuvant treatment or other treatment strategies including radiation, but none have demonstrated a survival advantage or improvement over the current standard of care.
