The timing of reoperation for incidental gallbladder cancer : Sooner or later?

Ethun and colleagues3 investigate this question using retrospective data from the multi-institution US Extrahepatic Biliary Malignancy Consortium database. They suggest that re-resection at 4 to 8 weeks following diagnosis (group B) optimizes long-term survival outcomes compared with reoperation at early (less than 4 weeks, group A) or late (more than 8 weeks, group C) intervals. The authors are to be commended for providing the largest analysis of incidental gallbladder cancer specifically evaluating the timing of reoperation, but the conclusions from their retrospective analysis should be met with caution.
