Adjuvant treatment with the human epidermal growth factor receptor 2 (HER2)-targeting monoclonal antibody trastuzumab in combination with cytotoxic chemotherapy has dramatically improved survival for patients with HER2-positive breast cancer. Trastuzumab inhibits the constitutive growth signaling in cancer cells driven by the HER2 oncogene; however, a preclinical study1 points to additional immune-mediated mechanisms required for its antitumor efficacy. Clinical trials also suggest an association of immune-related markers and outcome in untreated, neoadjuvantly or adjuvantly treated HER2-positive breast cancers (summarized by Bianchini and Gianni2).
