End of Methotrexate for CNS Prophylaxis in Diffuse Large B-Cell Lymphoma

The past 2 decades have seen remarkable improvement in outcomes for patients diagnosed with diffuse large B-cell lymphoma (DLBCL), largely because of novel therapies in both the up-front and relapsed settings.1,2 Despite these advances, CNS involvement either at the time of diagnosis or more commonly in the relapsed setting still portends a very poor prognosis. In a recently published analysis of over 1,000 patients with secondary CNS involvement of DLBCL were treated with highly aggressive therapies including stem cell transplantation, and 2 year progression-free survival was only approximately 40% for patients with CNS involvement at diagnosis and CNS-isolated relapses and 16% for synchronous relapses.
