From Bias to Evidence—The Rise of Stereotactic Radiosurgery in Brain Metastasis Treatment

In this issue of JAMA, Aizer and colleagues present the results of a randomized clinical trial demonstrating that patients with 5 to 20 brain metastases receiving stereotactic radiosurgery experience a lower symptom severity and less interference with daily functioning than patients who receive hippocampal-avoidance whole brain radiation. This study should be viewed from the perspective of where it fits with respect to studies published over the past 2 decades, as well as from the perspective of the evolving brain metastasis population, which has experienced a significant stage migration over the same period.
