Proud about PRADA: radiotherapy before deep inferior epigastric perforator flap reconstruction

In The Lancet Oncology, Paul Thiruchelvam and colleagues 1 report the findings of a prospective study (PRADA) that evaluated the safety and feasibility of skin-sparing mastectomy with axillary surgery and immediate autologous-based deep inferior epigastric perforator (DIEP) flap reconstruction after preoperative radiotherapy for patients with breast cancer. Over a period of almost 2 years, 33 patients were enrolled at two centres in the UK. The primary endpoint was the rate of open breast wounds greater than 1 cm requiring dressing or packing at 4 weeks after surgery. Secondary outcomes included the presence of an open breast wound at 8 and 12 weeks after surgery, DIEP flap failure rate, and patient-reported satisfaction with the breast reconstruction.
