High-quality colonoscopy decreases post-colonoscopy colorectal cancer. Colonoscopy quality metrics have long been endorsed, with adenoma detection rate providing the strongest evidence for protection against post-colonoscopy colorectal cancer. 1 , 2 Adenoma detection rate has increased over the past 20 years, due in part to the establishment of key performance indicators, 3 , 4 yet wide variability in adenoma detection rate between endoscopists still exists. Adenomas per colonoscopy is increasingly being recognised as a measure to better delineate the ability of the endoscopist to detect more than one adenoma and decrease adenoma miss rate; two large observational studies have shown that endoscopists who detected more adenomas per colonoscopy had lower post-colonoscopy cancer rates.
