Exposure Histories and Cancer Risk—What Can We Learn?

In this issue of JAMA Oncology, Li et al present findings from a large case-control study of the association of weight history with colorectal cancer risk. The primary analysis used weighted number of years with overweight or obesity (WYOs), calculated as excess body mass index (eBMI, defined as BMI [calculated as weight in kilograms divided by height in meters squared] − 25, truncated 0) accumulated from age 20 years to the index date. The authors observed a consistent association between WYOs and colorectal cancer risk, with seemingly more pronounced associations than similar analyses using age-specific eBMI or untransformed BMI measures. While these results are intriguing, it is important to examine the broader meaning and implications of such measures.
